EMPLOYERS' LIABILITY INSURANCE Under the Workers' Compensation Amendment Act, 1947 5 every emnployer is required to deliver an Employer 5 State mnent of Wages t0 the Statc Fire Insurance Oflce On the prescribed form not later than 30th April. This does not apply to mem- bers 0 [ insnrance associations whose mem bers have heen exempted by thie Compen satlon Court; Or t0 local authoritles which have heen exempted by the Com pensation Court) A person who is not 4I1 employer On 1st April, but who alterwards becomes an emplover, Is required to de liver 3 Statement 0f WVages within four- teen days 0f commencing 10 employ The Statement 0f Wages should he forwarded by post Or delivered in person by thi e Employer to the Branch 0Mce for the District 0f the State Fire Insurance Office. As soon 43 practicable after receiving the Statement the omfice will send to the employer an Assessment of Premium; The premium must be paid within one month after the date of assessment: Please apply to any State Fire District omnce, 01 t0 8 Post omce in any town, for & Statement of Wages Form
