NOTICE OF CHANGE OF SURNAME. ■ DAIS* MAGE DU GUARDE 9 TOOGOOD, heretofore called and known by the name of DAISY MAUD DU GUARDE COOK, of Kohiwai, Whareama, Wairarapa District, Dominion of New Zealand, Spinster, hereby give Public Notice that on the fourteenth day of September, 1909, I formally and absolutely renounced, relinquished, and abandoned the nee of my said surname of Cook, and thus assumed and adopted and determined thenceforth on all occasions whatso ever to use and subscribe the name of Daisy Maud Du Guarde Toogood in stead of the said name of Daisy Maud Du Guarde Cook, and I give further notice that by a Deed Poll, dated the fourteenth day of September, 1909, duly executed and attested and enrolled in the Supreme Court of New Zealand, at Wellington, on the fifteenth day of September, 1909 I formally and absolutely renounced and abandoned the «aid surname of Cook and declared that I had assumed and adopted, and intended thenceforth upon all occasions whatsover, to use and subscribe the name of Daisy Maud Du Guarde Toogood instead of Daisy Maud Du Guarde Cook, and so as to be at all times thereafter called, known, and described by the name of Daisy Maud Du Guarde Toogood exclusively.
