The Board of Enquiry into the recent railway accident near Mungaroa report that they consider driver Smith erred very gravely in judgment in taking the train too far down the grade before trying to stop it, and showed lack of decision in not acting promptly at a [critical moment. They also find that his judgment failed him when he endeavoured te quickly stop the train when he observed the derailed waggon.
