seriber shall fracture a leg above the ankle, or an arm above the wrist, or shall sustain a broken jaw or collarbone. CHILDREN’S ACCIDENTS (16) £3 will be paid to the parent whose name appears on the registration form in the event of an accident to his or her child over the age of six and under the age of 16 years causing broken arm above the wrist or broken leg above the anklet providing such accident is the direct result of one of the aforementioned and defined accidents and does not occur during employment. (f) That Medical Certificates and other necessary information be furnished by the claimant upon request.
