her private dwellinghouse, flat or rooms, such address being the address registered on the subscriber's certificate of registration, or in his or her garden attached to such dwelling-house, flat or rooms. (15) £lO if as the result of an accident above described the subscriber shall fracture a leg above the ankle, or an arm above the wrist, or shall sustain a broken jaw or collarbone. CHILDREN'S ACCIDENTS (16) £5 will be paid to the parent whose name appears on the registration form in the event of an accident to his or her child over the age of six and under the age of 16 years causing broken arm above the wrist or broken Teg above the ankle, providing such accident is the direct result of one of the aforementioned and defined accidents and does not occur during employment. PROVIDED ALWAYS that the above Undertaking and Agreement is subject to the following special conditions which are the essence of the contract, namely, (a) The scope of the Insurance is restricted to accidents happening in New Zealand and to steamers sailing within New Zealand territorial waters. (b) In the case of death, such death must result within 30 days after the accident. (d) IN THE CASE OF PERMANENT TOTAL OR PARTIAL DISABLEMENT that notice of the accident be given by the subscriber as aforesaid within fourteen (14) days of the accident. (e) IN THE CASE OF TEMPORARY TOTAL DISABLEMENT that notice of the accident be given by the subscriber as aforesaid within seven (7) days of the accident. (f) That Medical Certificates and other necessary information be furnished by the claimant upon request. (g) That the total liability in respect of claims arising out of any one railway
