through the loss of one limb or one eye. (b) In the case of death, such death must result within 30 days after the accident. (c) That notice of the death and the accident be given within forty (40) days after the accident by the subscriber or representative to THE SUN Newspapers, Ltd., Auckland. (d) IN THE CASE OF PERMANENT TOTAL OR PARTIAL DISABLEMENT that notice of the accident be given by the subscriber as aforesaid within fourteen (14) days of the accident. (e) IN THE CASE OF TEMPORARY TOTAL DISABLEMENT that notice of the accident be given by the subscriber as aforesaid within seven (7) days of the accident (f) That Medical Certificates and other necessary information be furnished by the claimant upon request.
