DIRECT QUESTIONS FOR EVERY MAN. REGISTER TO BE COMPLETED NEXT MONTH. The proclamation ordering the preparation of the National Register is to Ire issued on Tuesday, October 26th. The collection of the information is to be completed within, the succeeding fortnight, every man between the ages of seventeen years and sixty years being required to fill in a form and return it ur rite authorities orr or before November 9th. ft is estimated that there are 620,066 men in tiro Dominion of the specified ages, and the Re-gister-General's Department is securing 406,660 forms. Those forms will be classified when they have been collected orr a card-index system, and will constitute the .Register.
Tiro form is headed “New Zealand National .Register of Men.” Rack man is required to fill in his name, his place of residence, and his place of business, if any. Then ho must state the date of hi s birth, his age last birthday, and the place of birth of himself, his father and mother. The remaining questions are as follow; — 3. (a) Are you a British subject (1) by birth ? (2) by naturalisation ? (b) If naturalised, state (1) date of naturalisation; (2) place of naturalisation (e.g., United Kingdom, New Zealand, Canada, or as tiro case may be). (c) Arc you a foreign subject? If so, state country of allegiance. 4. (a) Stata whether single married (“M.”), or widower (“W”). (b) State number of persons dependent on you—(1) wholly dependent; (2) partially dependent. 5. (a) What is your present occupation ?
(!>) In connection with your present occupation, uro you—(is an employer of labor? or (2, working on your own account, bn; not employing laoor? or (3) assisting, but not receiving salary or wages? or (5) tii miployed for more than one week ? (c) If employed, state business of employer and name of employer. (d) Whas other occupation (if any) have you had practical experience of ? 6. (a) State (1) height; (2) weight. (b) State whether your general health is (1) very good; (2) good; (3) indifferent; (4) bad. If health indifferent or bad, state cause. (c) If suffering from blindness, deafness, loss of limb, or other physical infirmity, give particulars. / 7. If you have had military training at any time, state—(a) arm of the service; (b) rank held; (c) total length of service an

willing to become a member oi an expeditionary force if required? If so, in what arm of the service do you profox to serve. (c) li you have not volunteered fo;: military service and are no prepared to volunteer, are'jo: willing to servo in any civil capacity in connection with the present war? If so, state in what capacity. (d) If there are any circumstances or reasons that prevent you from offering your services, state them.
Section 9 provides for iuformat'ou regarding relatives wholly or partially impendent on the person who in registering, it asks their name and address, age, occupation (if any) or condition (e.g., retired, old-age pensioner, apprentice, invalid, or as tho case may be); relationship (e.g., wife, sou, mother, or as the case may bo); 1 whether wholly or partially dependent; iu caso of partial dependents, extent of dependency per annum; other income (if of dependent per annum. At tho end of the card us a lino for certifying that all particulars given in tho schedule are correct, and the intimation that all information will bo regarded as strictly confidential. Each copy of tho schedule ig to bo accompanied by an envelope, and tho man who lia s filled it in is required to hand it to an officer of the Post and Telegraph Department, or post it to the Government Statistician at Wellington. Tho general instructions accompanying the form make the following additional provisions: Any person to whom the Proclamation relates who wilfully fails to furnish particulars as required, or who wilfully makes any false statement with respect to any such particulars, is liable to a fine of £IOO or imprisonment for six months. Every officer employed in the collections of the schedules or in the compilation who wrongfully divulges or makes use of any information acquired by him in tho course of hie I duties is liable to a fine of £IOO. | Tho register to be compiled under the above Act is purely a civil register for the purpose of ascertaining tho resources of the Dominion in men and the registration here re- ! quired does not involve enlistment nor is it in substitution for the present system of enlistment. Those desirous of enlisting can obtain enlistment forms at the .nearest post office or Defence Office.
