Every policy-holder, who is such in respect of a policy granted or entered into in New Zealand by a Company who shall have made a deposit of securities under the Life Assurance Companies Act, 1873, may, if the policy or contract has been made before the commencement of the said Act, register such policy with the Public Trustee within twelve months after the commencement of the said Act, but not after ; and if made after the commencement of the said Act, then within six months after the making thereof. (N.B. —The Ist of November, 1873, is fixed as the commencement of the Act.) 2. The person intending to register a policy shall either deliver the same to the Public Trustee at his office in Wellington, or may deliver the same to an agent; and every such person shall make application to register such policy in a form which can be obtained at any Post Office in the colony at which Money Orders are issued. 3. The Public Trustee shall make and keep a book for the registration of policies, in which the particulars of each policy shall be recorded, 4. There shall be paid to the Public Trustee by the policy-holder a fee of five shillings on the registration thereof. 5. Each policy shall bear a registration number according to the order in which it has been received, and when registered the Public Trustee shall make and sign a memorandum thereon in the form or to the effect following:— No. Registered in pursuance of the Life Assurance Companies Act, 1873, this day of 18 .
